  • Overview

    The Palliative Care Physician will provide inpatient consultations, act as a medical resource to the interdisciplinary group, assure continuity of hospice medical services, and assure symptom management. S/he will serve as a hospice champion promoting and representing the program to physicians, discharge planners, and community health organizations.

    Responsibilities

    • Provide inpatient palliative care medicine consultations for symptom management
    • Counsel patients and family members regarding symptom management, optimal goals of care and location of care
    • Participate in a multidisciplinary care team and act as a medical resource to the team
    • Ensure the coordination of care across settings by improving communication among providers, particularly as the patient’s needs change.
    • Assure continuity of hospice medical services
    • Advocate for the role of palliative care medicine and services
    • Serve as a hospice champion promoting and representing the program to physicians, discharge planners, and community health organizations
    • Participate in the medical student, medicine residency and hematology-oncology fellowship teaching programs
    • Participate in performance improvement activities regarding palliative care medicine
    • Control pain, alleviate psychosocial distress, address spiritual issues and meet practical needs throughout the continuum of care.
    • Improve access to information for patients and families so that they understand the patient’s condition and treatment options.
    • Coordinate care across settings by improving communication among providers, particularly as the patient’s needs change.

    Qualifications

    Education:

    • Graduate of a medical school accredited by the Council on Medical Education.
    • Completion of an accredited residency program.          

    Experience:

    • Prior hospice experience required.

    Licenses, Certifications:

    • License to practice medicinein New York State.
    • Certification in Hospice and Palliative Care Medicine.
    • Hospice and Palliative Care Fellowship preferred.
    • Has successfully completed the credentialling process at the Hospital facility.

    Physical Requirements:

    • Position requires prolonged periods of standing, reaching, walking throughout the working day. 
    • Position will be required to stoops, bends, lifts, carry items weighing up to 75 pounds.
