  • Overview

    Performs social work duties, which include but not limited to, helping the registrant and/or his caregiver with social, emotional, economic and rehabilitation problems.  Working with the Interdisciplinary Team in planning care and providing intervention through direct services. May be assigned to supervise undergraduate or graduate social work students or volunteers.

    Responsibilities

    • Initiates the admission process; takes all inquiries, assesses potential registrants, conferences with Director and Nurse to determine appropriateness for Adult Day Health Program, and initiates the record where required.
    • Provides direct social work services and counseling to registrants, families/caregivers (individually or in a group setting as appropriate).
    • Participates in the bi-annual Interdisciplinary Care Plan Meeting or sooner if required.
    • Maintains community resources file and makes appropriate referrals for registrants or caregiver as requested.
    • Provides education to registrants on Advance Directives and assists them complete them appropriately.
    • Provides regular education programs to registrants.
    • Provides in-service programs to registrants.
    • Attends in-service programs as required.
    • Maintains registrant’s clinical record every 90 days or as needed.
    • Maintains statistical data for the Director.
    • Prepares monthly/annual reports to the Director.
    • Represent the Schulman and Schachne Institute Adult Day Health Care Program at the professional and community meetings.
    • Makes presentations on Schulman and Schachne Institute Adult Day Health Care Program to community agencies as requested.
    • Assist registrant/family/caregiver with entitlements (Medicaid, SSI, HEAP, Food Stamps, special telephone service, etc.)
    • Makes home and hospital visits for registrant evaluation as well as instruction to caregiver or other health professional.
    • Perform other duties as assigned.
    • Maintains good attendance.
    • Demonstrates safety in the performance of their job responsibilities and duties.

    Qualifications

     Education: 

    • MSW from an approved university.
    • LCSW preferred.

     Experience: 

    • At least one year experience in Long Term Care; Adult Day Health Care preferred.

    Knowledge and Skills:

    • Demonstrates social work knowledge and possesses skills to integrate this knowledge into social work practice.
    • Demonstrates leadership ability and serves as a professional role model to staff.
    • Demonstrates effective interpersonal skills with registrants/families/caregivers, Interdisciplinary Team and community professionals.
    • Demonstrates effective case management skills with registrants/families/caregivers, Interdisciplinary Team and community resources to maintain registrants emotional wellness and safety.

    Licences and Certifications:

    • Registered as a Licensed Clinical or Certified Social Worker (LCSW) with the New York State Education Department.
    • ACSW preferred.

    Physical Requirements:

    • Consistent with that necessary to work in a variety of patient care and office settings. 
    • Position requires prolonged periods of standing, reaching, walking throughout the working day. 
    • Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
